USS Scorpion (SSN-589) was lost 22 May 1968 with ninety-nine dead; a Navy Court of Inquiry could not ascertain certain cause. Later FOIA releases of inquiry and advisory materials, plus ONI-associated acoustic analyses (e.g., Rule), contest explosion and mechanism readings. Soviet-attack folklore is labeled contested. Contested/L2 keeps Navy doors primary without sealing a single cause.
